Office Policies

At our practice, we are committed to providing safe, ethical, and high-quality psychiatric care. The following policies are in place to ensure respectful communication, consistent treatment, and a professional experience for all patients. We kindly ask that all patients review and follow these guidelines.

Appointments

Appointments are scheduled during business hours by phone or in the clinic.

Patients are responsible for attending appointments on time and maintaining regular follow-up care as recommended by their provider. Consistent communication is expected to ensure continuity of treatment.

Cancellations, No-Shows, and Late Arrivals

We require at least 24 hours’ notice to cancel or reschedule an appointment.

Missed appointments without notice are considered no-shows, and repeated no-shows may impact your ability to schedule future appointments.

If you arrive more than 10–15 minutes late, your appointment may need to be shortened or rescheduled.

Follow-Up Appointments

Follow-up appointments are required to monitor progress and adjust treatment as needed. Most patients are seen every 4–6 weeks. Patients prescribed stimulant medications (Adderall, Vyvanse, Ritalin, etc.) or other controlled medications may be required to be seen every 30 days in accordance with safety guidelines and state regulations.

Telehealth & Treatment Eligibility

Telehealth appointments are available for established patients when clinically appropriate and at the provider’s discretion. Patients must be physically located in the state of Florida at the time of the telehealth visit and may be asked to verify their location. Stable internet connection, a private environment, and video capability are required for all telehealth sessions.

Certain conditions and medications may require in-person evaluation before treatment can begin. Controlled medications, including stimulant medications used for ADHD, may not be prescribed during a telehealth visit unless all legal and clinical requirements are met. Patients who are not actively engaged in treatment, fail to comply with follow-up appointments, or do not meet clinical criteria may become ineligible for continued care through our office.

Medical Records Requests

Patients may request a copy of their medical records in person during office hours by submitting a signed medical records release form. Records are processed in accordance with HIPAA privacy regulations and may take up to 10 business days to complete.

In compliance with Florida state guidelines, there is a copying fee of $1.00 per page for the first 25 pages, and $0.25 per page for each additional page. Fees must be paid before records are released.

 

For privacy and security reasons, medical records cannot be sent by email. Records will only be released through approved secure methods, including:

​

  • Secure fax to another provider or organization

  • Encrypted upload through a secure portal (if available)

  • In-office pickup with valid photo ID

There is no fee for records that are sent directly to another medical provider for continuity of care when a signed release is provided. Records will not be released to any third party without patient consent unless required by law.

Letters, Forms, and School Documentation

Our office can provide clinical letters and documentation when medically appropriate. This may include verification of diagnosis, treatment summaries, and school-related documentation such as 504 Plans, IEP support letters, or accommodations letters.

All requests must:

  • Be submitted by the patient or legal guardian

  • Be related to active treatment with our provider

  • Allow up to 7–10 business days for completion

  • Be clinically appropriate based on evaluation and records

​

Diagnostic letters and school documentation will not be provided at the first appointment and require adequate clinical history and documentation to support the request.

​

​There is a $45 administrative fee for each letter.

Diagnosis Confirmation Letters

Provides verification of a patient’s mental health diagnosis based on clinical evaluation and treatment history. Often used for academic, workplace, or personal documentation purposes.

​Educational Support Letters (School, 504, and IEP)

We provide letters to support educational accommodations when clinically appropriate. These letters may be used for school accommodation requests, 504 Plans, or Individualized Education Programs (IEPs). They describe how a student’s mental health condition may impact learning, concentration, behavior, or academic performance and outline recommended educational supports.

Emotional Support Animal (ESA) Letters

(based on clinical eligibility)

Written only when clinically appropriate, this letter explains the therapeutic benefit of an Emotional Support Animal for a diagnosed mental health condition. ESA letters require documented treatment history and follow clinical guidelines.

Bariatric Surgery Psychiatric Clearance Letters

(based on clinical eligibility)

Evaluates mental health readiness for bariatric surgery and provides psychiatric clearance when appropriate. This includes assessment of stability, treatment compliance, and readiness for lifestyle changes.

Jury Duty Excusal Letters

Provided only when a mental health condition significantly impairs ability to serve and is clinically justified.
